Download – (be sure to save the PDF to your desktop or documents folder) then fill in the PDF form with acrobat reader, selecting and placing in your information and save the file. Email the completed form/s to us at : register@merivalepap.co.nz, along with your proof of payment – and be sure to add in who referred you so that their name can also go into the draw! Payment is required when registering.
You don’t need to sign the form until you collect your gear.
PLAY RUGBY & WIN
There are multiple ways to win this season playing for the Merivale Papanui RFC – on the field, at registration or at one of the weekly prize givings.
• Get yourself and a friend into the draw for a Wi-Fi Samsung Galaxy S when you refer a friend to register*
• Get into the draw for a $20 iTunes card when you register* and “Like” us on facebook.
• Complete the PDF register* form, email it back and pay before 29th Feb 2012 and get into the draw for a ‘Sideline’ Signature book.
* ENTRY RULES
To successfully enter the prize draws, you need to:
a) complete a Merivale Papanui RFC 2012 registration form and have paid the subscription fees; and
b) to be in the draw to win the SAMSUNG GALAXY S Wi-Fi 5.0 YP-G70 you must have referred a friend (who has completed a registration form and paid their subscription fees). This will entitle you and your friend to both gain entry into the draw); and or
c) to be in the draw for one of the 5 $20 iTunes cards, you are required to have “liked” the Merivale Papanui Rugby Football Club Official Club Site on Facebook; and or
d) to be in the draw for a Canterbury Rugby Football Experience ‘sideline’ signature book you must have completed the registration process via the PDF before the 29th Feb 2012.
